Full Circle Home Care (FCHC) provides services to individuals with developmental disabilities who are residents of Anne Arundel County, Baltimore County, Baltimore City, Harford County & Howard County. 


FCHC proudly provides Developmental Disabilities Administration Services:

  • Personal Support
  • Nursing Support Services
  • Community Development Services


We are dedicated to supporting individuals with developmental disabilities in achieving greater independence, reaching their personal goals, and thriving within their communities. Our comprehensive services and programs are designed to enhance quality of life, promote inclusion, and empower each individual to live fully and confidently. 



Personal Supports (DDA)

We provide personal support services in both the home and the community to assist individuals living in their own home or with family.


Our goal is to help each person acquire and strengthen the skills needed to maximize personal independence. These supports go beyond assistance with daily living activities and include a broad range of services designed to promote self-sufficiency, skill development, and active community engagement. 


Our services include:

  • Support with daily personal care tasks
  • Skill building for independent living and community integration
  • Medication management and administration
  • Development of community safety and survival skills
  • Assistance with errands and grocery shopping
  • Guidance on using public transportation
  • Encouragement and support for participating in social and recreational activities



Community Development Services (DDA)


Through our Community Development Services (CDS) program, we support individuals in building and sustaining the skills needed for greater independence and active participation in their communities.


CDS focuses on meaningful engagement, including interactions with individuals without disabilities, attending community events of their choice, and developing social and daily living skills. Based on each individual’s interests, abilities, and goals, we also provide support for volunteer opportunities and travel training to further enhance community integration and personal growth. 

 We achieve this by providing: 

  • Activities that foster community inclusion and integration based on the participant’s interests
  • Access to resources for travel and transportation 
  • Support and resources to strengthen self-advocacy skills
  • Encouragement and support to attend and participate in local community events
  • Opportunities to volunteer with nonprofit organizations aligned with the participant’s values and interests
  • Opportunities to engage in paid or unpaid internships that provide real-world experience and skill development



Nursing Support Services (DDA)


Our Registered and Delegating Nurses (RN/DN) are dedicated to delivering personalized nursing care to the individuals we serve. They perform thorough health assessments, routinely monitor medication administration, review Health Risk Screening Tools (HRST), and more. These efforts prioritize the health and safety of each participant, helping to prevent adverse outcomes. 

Our nursing services include:

  • Nurse Consultation Services
  • Nurse Health Case Management
  • Nurse Case Management and Delegation of Services

The DDA focuses on six (6) areas when thinking about services. We make sure that services support: ​

Self-Determination, and the belief that  people can: 

  • Plan their own lives 
  • Make their own decisions 
  • Plan and choose their own supports  
  • Decide how to spend the funding they have for services 
  • Take responsibility for the decisions they make ​

Self-Advocacy, and the commitment to encourage people to: 

  • Speak up for the things that are important to them 
  • Ask for what is needed or wanted and tell others about thoughts and feelings 
  • Speak up for their rights and make choices and decisions 

Supporting Families, and the development of services and supports that: 

  • Make the most of the strengths and abilities of families to support their family members 
  • Connect families to support each other 
  • Increase access to resources, support and services 

Independent Living, and to make sure that people have: 

  • The choice of where and how to live 
  • Opportunities to use and learn new skills 
  • The option to live like people without disabilities 
  • Opportunities to make decisions about their life 
  • The ability to get involved in activities they choose

Employment, and the commitment to: 

  • Be an Employment First state 
  • Use each person’s strengths, interests, and skills to support competitive integrated employment  

Assistive Technology​, and the commitment to: 

  • Be a Technology First state
  • Provide support and assistance for people to:
    • Explore technology to help meet their needs  
    • Increase independence 
    • Become more connected to their communities​
