Time Sheet

This paper time sheet should only be used if you have tried clock in or out via app or mobile call in or out.

I hereby certify that the above stated hours are an accurate record of my working time. I also certify that no injury was incurred by me during this time. I certify that hours worked by Full Circle Home Care employee are correct.

Your appearance and behavior reflect on the company. Cleanliness and good grooming are required.

Field Uniforms

· Company T‑shirt with pants/capris and company badge. First order must include at least 2 shirts. 

· T-Shirt Cost: $22 (S–XL); $25 (2XL–3XL). T‑shirts may take up to 2 weeks.

· While waiting, wear a plain blue t‑shirt (no rips/logos/phrases).

· Pick up shirts in office or they can be mail.

· Supportive non‑slip shoes required.

· First order must include at least 2 shirts. 

Request Your Full Circle ID Badge Requirements

• Must be an active Caregiver with Full Circle Home Care.
• Must have credential paperwork update to date.
• Must have training  documentation up-to-date.

Photo Requirements

• Plain Background
• Image from top of head to shoulder
• Good lighting

Replacement ID badge fee: $10. Attach a white‑background headshot. This may take 5-7 business days to complete.

Request Options:  

A written request describing needed documents.

· Processing time: up to 7 business days; past‑employee records in storage may take up to 14 days.

· Printed copies: $1 per page. Emailed copies: free.

Do not come to the office in-person requesting information, as request will be denied.
